TELEPHONES UPDATE

You may be aware that we experienced major problems with our telephone system yesterday (14th November).  The system appears to be functioning correctly now.  We apologise for any inconvenience you may have experienced yesterday.

 

The Best GP Surgery in Sheffield - As Rated by You Once Again!

We are absolutely delighted at the news that you have rated us as the best GP Surgery in Sheffield in the GP Patient Survey carried out between January and March 2018.

New Patient Registration - Please see the New Patients tab on the right hand side of the screen for registration forms.

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception. You will need to provide identification and proof of address.

The medical centre serves the surrounding areas of S7 and S11

For patients not already registered with a GP in the UK we also require proof of residency. Please inform the receptionists if you have any long standing medical problems and they will arrange for you to see a doctor if appropriate. It may take time to obtain your medical records but you will still be able to see a doctor.

If you are on repeat prescription and require a prescription before we receive your records it would be helpful if you presented the list last prescribed by your previous GP.

Change of Name or Address

Please let us know promptly of any changes to your personal details. We do text out appointment reminders so please let us have an up to date mobile number. If you do not wish to receive reminders by text please let us know.
